The “Registered Nurse (RN)” plans, organizes and directs patient care and is experienced in nursing, with emphasis on community health education/experience. The RN is responsible for providing clinical nursing care and coordination of all medical services for individuals. This professional nurse builds from available resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Qualifications
- Current and unrestricted Registered Nurse licensure
- Home Health RN (Per Diem)
The PD RN is responsible for providing clinical nursing care on an as needed basis.

- Current and unrestricted Registered Nurse licensure
- Certified Home Health Aide (Part time)
The hospice aide works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient’s personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed.
Qualifications
- Home Health Aide (HHA) license – required
- Physical Therapist
The Physical Therapist will work to provide care to patients as ordered.
